I arrived at the premise at 9:05 am, proceeded to the decon, signed the log book, don the appropriate level of personal protective equipment to enter the regulated area on 2"‘’floor. One person was present for the decon maintenance, cleaning the shower chamber. I used B stairwell to proceed to the 3"* floor.

3'“ Floor - 33 workers were present on the floor, cutting metal fixtures from the ceiling, spraying around the area with amended water. Carpenters working with plywood, building the platforms across shafts. Electricians setting up electrical room, plasticizing inside with 6 mil poly, wrapping the cables. Workers on scissor lifts at the south extended area were removing insulation and taking down air ducts; cutting metal fixtures.at the south side decon, were cutting air duct to remove and cut into the pieces. 4 Gaylord boxes were ready for load out. Inspection was done on micro trap machines and tents. I used B stairwell to proceed to the 2'"’ floor.

2"“ Floor - 33 workers were found inside the work place, cutting and removing black metal panels from the columns at the east side work area. Workers on scissor lifts at the south/east side vestibule were cutting metal fixtures from the ceiling. Electricians inside the electrical room were cutting dead pipes and plasticizing the room with poly. Workers demolishing the brick wall at the south/east side, spraying around the area with amended water, picking up debris, placing into the black asbestos bags; cutting metal using tools attached to the HEPA vacuum, others spraying the area with amended water, wrapping beams with poly, preparing boxes. 3 Gaylord boxes were on the floor sealed, sprayed and ready for load out. I used B stairwell to proceed to the V floor.

1“ Floor - 42 workers on the floor. Electricians were working with the wires at the electrical panel. Workers ion scissor lifts were removing and drop ceiling, spraying with amended water; clearing the shafts from debris, placing into the boxes and spraying with water, cutting metal at the various locations, preparing, wrapping and loading out boxes through the waste decon. A large amount of debris and poly wrapped beams exists on the floor. 8 Gaylord boxes were on the floor ready to be taken out. Inspection was done on micro trap machines and tents. I used B stairwell to go back to the S'^floor.

After following the decontamination procedure I signed the log book and used south hoist to return to the ground floor. Inspection was done on ATC trailer.
