I arrived at the premise at 10:00 am. LVI schedule informed that activities include: scraping Atrium area on 2"'' floor, clear floor, cut ducts, pipes, scrape fireproofing South East side. Eagle scaffolding - erect platforms in various shafts, loading out boxes to ATC trailer on 1®‘ floor.

I proceeded to the decon, signed the log book, don the appropriate level of personal protective equipment to enter the regulated area on 3"’loor. One worker was present for the decon maintenance, cleaning and organizing the clean room. Negative air pressure on the floor was -0.030. I used the stairs to proceed to the 2"'' floor.

2'"' Floor - 18 Asbestos Handlers were present on the floor. Workers were found on scaffolds scraping the beams and vertical columns of the area 9 - Atrium Area - from the fireproofing, washing scaffolds and equipment, spraying the fireproofing with amended water. Workers on the floor were sweeping the floor using equipment equipped with HEPA filters, cleaning the floor from debris, placing into the boxes; plasticizing the area 10, moving scissor lifts, preparing for fireproofing removal. Others were replacing the filters of micro trap machines, preparing and loading out boxes. 12 sealed Gaylord boxes were on the floor, sprayed and ready for load out. 22 micro trap machines were powered on and 5 on stand by. Inspection was done on micro trap tents. I used the stairs to proceed to the 1^' floor.

1®* Floor - 64 workers were found on the floor. Workers on scaffolds at the area 1 - south side - were scraping beams and columns from fireproofing. Others were cleaning the floor from debris, placing into the boxes. Eagle scaffolding workers were erecting platforms inside the north/west shaft. At the north/east side area workers on two scaffolds were cutting air duct, using equipment equipped with HEPA filters, demolishing and lifting down the pieces, spraying area with amended water. On north/west side of the floor workers were plasticizing the critical barriers. Others were cutting metal at the north/east, wrapping beams with poly, preparing and loading out boxes through the waste decon. 15 Gaylord boxes were on the floor ready for load out and 15 more boxes were still open. Inspection was done on micro trap machines and tents.

I observed that exhaust duct of the micro trap machine near the tent B-5 was cut off. Foreman Paul on the floor was advised, and he immediately directed workers to repair the exhaust duct. I used the stairs to precede to the 3"'* floor.

After following the decontamination procedure I signed the log book and used south hoist to return to the ground floor. Inspection was done on ATC trailer.
