I arrived at the premise at 1:35 pm. LVI schedule informed that activities include demo, cut raceways, box debris, S’” floor demo shaft on 8"' & T'" floors, demo, box debris on 6*'' floor, prep micro trap tents, install traps on 5'’’ floor.

I proceeded to the decon, don the appropriate level of personal protective equipment to enter the regulated area on 8'” floor. Two workers were cleaning decon. Negative air pressure on the floor was 0.060.

8"’ Floor - 34 workers were present on the floor. Workers on 5 scaffolds on north/east side of the work area were cutting raceways with the power tools attached to the HEPA vacuum, spraying with amended water, cleaning the floor from debris, electricians inspecting the electrical panel. Workers were cutting metal into the pieces using electrical tools attached to the HEPA vacuum, removing the metal fixture from the ceiling, wrapping electrical cables with poly. One worker was working with his arms exposed. He was told to cover his arms with cleaves and duct tape. Other workers were preparing and wrapping boxes for debris. 15 Gaylord boxes were ready for load out. Inspection was done on micro trap machines and micro trap tents. Air monitoring pumps were powered on. I used B stairs to proceed to the 7th floor. 7 Floor - 18 workers were found on the floor in the process of demolishing air duct, cutting pipes into pieces, using tools with HEPA vacuum attached, carpenters setting up the platforms across the shaft area, others preparing and wrapping boxes, lifting up the electrical cables. 22 Gaylord boxes were ready for load out. 20 micro trap machines were powered on and 5 were on stand by. Inspection was done on micro trap tents. I used B stairs to proceed to the 7*'’ Floor. 6"’ Floor - 18 workers were working on the floor, demolishing the air duct and cutting into the pieces with power tools attached to the HEPA vacuum, spraying with amended water around the area, demolishing the sheetrock from the wall inside the shaft area, cleaning the floor, placing debris into the boxes and spraying. Workers were wrapping beams with poly, preparing boxes; a large amount of poly wrapped beams exists on the floor. 5 Gaylord boxes were ready to be taken out. I observed that fire door between two decons - waste and personal - was not sealed from inside the work area. I met LVI supervisor at the job site and advised him to seal the door with blue tape. Inspection was done on micro trap machines and tents. I used B stairwell to proceed to the 5**’ floor. 5"’ Floor - 12 workers were present on the floor. Workers were preparing micro trap tents, to install the traps, checking critical barriers, getting ready to work in electrical room. I used B stairwell to go back to the 8**’ floor, proceeded to the decon, showered out and signed the log book.

I used the north side hoist to get to the ground floor. Inspection was done on ATC trailer.
