I arrived at the premise at 8:45 pm. LVI schedule informed that activities include demo ducts, cut metal on 3™ and 2"^ floors, clear off floor for demo on 1®' floor.

I proceeded to the decon, signed the log OTji: , don the appropriate level of personal protective equipment to enter the regulated area on 1®* floor. Negative air pressure on the floor was 0.021. Three workers were present for the decon maintenance. Using B stairwell I proceeded to the 3'^oor.

3"* Floor - 8 workers were working on the floor at the south side of the work area, cutting metal duct with the power tool attached to the HEPA vacuum, picking up debris, placing into the boxes and spraying with amended water, preparing and wrapping boxes. A large amount of poly wrapped beams exists on the floor. 10 Gaylord boxes were ready for load out. Inspection was done on micro trap machines and tents. I used B stairwell to proceed to the 2'^'^ floor.

2"'“ Floor - 20 workers were found inside the work place, cutting air ducts into the pieces at the various locations on the floor, picking up debris, placing into the boxes and spraying. Workers on scaffolds were removing metal fixtures from the ceiling, others were wrapping beams with poly, preparing boxes, 23 Gaylord boxes and a large amount of poly wrapped beams were ready to be taken out. Inspection was done on micro trap machines and tents. I observed that filters of 6 micro trap machines were dirty: Handler Supervisor was advised and 1 worker replaced the filters of micro traps. I used B stairwell to proceed to the 1*‘ floor.

1’* Floor - 20 workers were clearing off the floor. A large amount of debris exists on the floor. Workers were clearing the shaft from debris at the north/west side of the floor, placing debris into the boxes, spraying boxes and work areas with amended water, replacing filters of the micro trap machines, preparing, wrapping and loading out boxes through the waste decon, preparing the work area for demolition. 28 open and sealed Gaylord boxes were on the floor. Inspection was done on micro trap machines and tents.

After following the decontamination procedure I signed the log book and left the remote decon, located on the ground floor. Inspection was done on ATC trailer.
