I entered the premise at 8:55 am. LVI schedule informed that activities include: clear floor, cut ducts, demo walls, ceilings on 1®' floor.

I proceeded to the decon, signed the log book, don the appropriate level of personal protective equipment to enter the regulated area on Vfloor. Two workers were present for the decon maintenance, organizing the clean room. Negative air pressure was -0.026.

V Floor - 75 workers were found inside the V floor work area. Eagle scaffolding workers at the north/west side extended area were erecting scaffolds, preparing the area to cut the vertical beams and metal fixtures from the brick wall.

Workers on mezzanines of the west side area were clearing off floor, picking up debris, placing into the boxes and spraying with amended water, preparing the area for air duct removal.

Eagle scaffolding workers were at the north side shaft area erecting the scaffolds. Workers were observed on scaffolds at the north vestibule removing insulation of the airducts, spraying with amended water during removal.

Other workers were: cutting metal into the pieces at the south side, spraying the area with amended water; preparing boxes, wrapping beams with poly, loading out sealed and sprayed boxes through the north side waste decon. Approximately 30 Gaylord boxes were ready for load out. 35 micro trap machines were powered on and 5 on stand by. Inspection was done on micro trap tents.

After following the decontamination procedure I signed the log book and left the decon. Inspection was done on ATC trailer.
