Contact Person Time of Inspection from:
Arnel Javal, Sr. Proj. Mgr/TRC Environmental 10:00 am to 11:15 am
I entered the premise at 10:00 am. LVI schedule informed that activities include:
Clear floor, cut ducts, demo walls, ceilings on 1’* floor.
I met Mamdouh Melbahary Project Scientist of TRC at the work site. We proceeded to the decon,
signed the log book, don the appropriate level of personal protective equipment to enter the
regulated area on Vfloor. Two workers were present for the decon maintenance,
cleaning shower chambers and organizing the clean room.
Negative air pressure was -0.028.
1“ Floor - 75 workers were present inside the work area. I observed that 5 workers working
inside former computer storage room were harnessed in scissor lift demolishing the drop ceiling;
amended water has not been used while demolition.
Two workers on the north/east side of the work area, harnessed in scissor lifts, were
demolishing the brick wall; other workers on mezzanine demolishing drop ceiling, cutting
metal nets using the power tool with HEPA vacuum attached, spraying with amended water during
demo. Foreman was advised and directed worker to wet the area with water.
On mezzanines at the various locations workers were demolishing the brick wall and
spraying with amended water, cutting and removing air ducts, breaking down hard drop ceiling,
spraying with amended water, placing debris into the boxes.
Other workers were: cutting metal into small pieces at the various locations, placing into the
boxes and spraying; wrapping beams with poly, loading out sealed and sprayed boxes through
the north side waste decon. A large amount of debris still exists on the floor.
Approximately 35 Gaylord boxes were ready for load out. 35 micro trap machines were powered
on and 5 on stand by. Inspection was done on micro trap tents.
I observed that: 1. Main layer of one of the exhaust ducts, connected to the micro
trap at the south side critical barrier, was broken and needed to be repaired; 2. Air
monitoring pump inside the tent was unplugged; two light bulbs were not protected - cages were
open. 3. Amended water inside the computer storage room was not used while demolition; 4. Two
workers were working with their wrists exposed while using power tools to cut the metal fixtures
from the wall and ceiling, Workers were advised to cover their wrists with sleeves and duct tape.
I met Jose LVI Supervisor to advise him to: 1. conduct emergency repair of exhaust duct,
which was repaired immediately; 2. Contact Platinum Electrician for immediate protection
cages repair.
After following the decontamination procedure I signed the log book and left the decon.
Inspection was done on ATC trailer.
