Contact Person Time of Inspection from:
Arnel Javal, Sr. Proi. Mqr/TRC Environmental 5:30 pm to 6:10 pm
I entered the premise at 5:30 pm. LVI schedule informed that activities include:
Clear floor, demo & wrap metal, load out on 1 floor.
I proceeded to the decon, signed the log book, don the appropriate level of personal
protective equipment to enter the regulated area on VTloor. Two workers were present
for the decon maintenance, cleaning shower chambers and organizing the clean room.
Negative air pressure was -0.031.
1®' Floor - 30 workers were found inside the work area. Two workers on mezzanine at the
north/west side work area were cleaning off the floor, picking up debris, metal pieces,
placing into the boxes and spraying with amended water.
Two workers were cutting metal into small pieces using equipment equipped with HEPA filter
at the north/east side, another worker was spraying with water around the area.
A large amount of debris exists on the floor. Workers at the south side area were wrapping beams
with poly. Others were replacing filters of micro trap machines, preparing, wrapping and loading
out sealed and sprayed boxes through the north side waste decon.
18 Gaylord boxes were ready for load out. 35 micro trap machines were powered on and
5 on stand by. Inspection was done on micro trap tents.
I observed that: 1. Main layer of the exhaust duct, connected to one of the micro traps at the
south side work area was broken and needed to be repaired;
2. HEPA vacuum was attached to the electrical tool, but was not powered on,
while the worker was cutting metal into the pieces at the south side work area
Supervisor was informed, who then powered on the HEPA vacuum
immediately.
3. First main layer of poly inside tent located at the north/east side area
was detached from the ceiling of the tent and was hanging down.
I met Julio Valero Handler Supervisor inside the work area to advise him to:
1. Conduct emergency repair of exhaust duct, which was repaired immediately;
2. Re-tape poly inside the tent - Supervisor directed two workers to repair the tent in concern;
3. Advise handlers to use equipment properly - Usage of HEPA vacuum during cutting operations.
Photos has been taken and attached to the Inspection Report.
After following the decontamination procedure I signed the log book and left the decon.
Inspection was done on ATC trailer.
