I entered the premise at 10:15 pm. LVI schedule informed that activities include: Clear floor, demo & wrap metal, load out on 1“* floor.

I proceeded to the decon, signed the log book, don the appropriate level of personal protective equipment to enter the regulated area on 1’‘floor. Two workers were present for the decon maintenance, organizing the clean room. Negative air pressure was -0.031.

r‘ Floor - 30 Asbestos Handlers were present at the work area. I observed two workers harnessed in scissor lift removing metal panels and bricks at the 2"“ vertical beam of the Hallway between shafts at the north/west side work area, spraying with amended water, picking up debris and placing into the boxes. At the left side of the entrance of west side personal decon two workers in scissor lift were cutting and demolishing air duct, using equipment equipped with HEPA filters, spraying around with amended water. There was a large amount of debris - mixed beams, metal fixtures and conduits - on the north/west side area of the floor. Two workers were spraying beams and wrapping with poly. Workers were cutting metal into small pieces using equipment equipped with HEPA filter at the north/west and south side work area, spraying with amended water. Others were preparing, wrapping and loading out sealed and sprayed boxes through the north side waste decon. lOGaylord boxes were ready for load out. 35 micro trap machines were powered on and 5 on stand by. Inspection was done on micro trap tents.

I observed that: 1. Exhaust duct of one of the micro trap machines at the north side area was partially detached and needed to be re-taped. 2. Electrical cables were hanging down at the south side work area. Handler Supervisor Julio Valero was advised, and tree workers performed emergency repair of the exhaust duct and elevation of the electrical cables.

After following the decontamination procedure I signed the log book and left the decon. Inspection was done on ATC trailer.
