I arrived at the premise at 8:30 am. LVI schedule informed that activities include clean work area - North vestibule on 19“’ floor, prep area, cut walker ducts, raceways, clean work areas - South Comp Bay on is'” floor, scrape areas 11-13, box debris. Eagle putting platform across shafts on 11*“ floor, clear off floor, move debris on 10*" floor. I used north hoist to go to the IB*" floor, I proceeded to the decon, followed all precautions necessary and left decon; using stairs I went to 19" floor.

19"’ Floor - 5 workers were on scaffolds inside the work area, cleaning the ceiling, beams. I observed 3 holes on ceiling; supervisor was advised to cover the holes. Three micro trap machines were powered on. I proceeded to the decon, followed all precautions necessary and using stairs went to 15" floor.

15" Floor - 5 workers were on 2 scaffolds cutting raceways, spraying with amended water, 5 asbestos waste bags were on the floor ready for load out Three holes were observed on the ceiling; supervisor covered them with duct tape, 3 micro trap machines were powered on. After following the decontamination procedure I signed the log book and used north hoist to go to 10" floor. I went to the decon on 10" floor, signed log book, donned personal protection, left decon and using B stairs went to 11" floor.

11‘" Floor-20 workers were in a process of scraping fireproofing in the areas 11-13, plasticizing, cleaning the platform, placing debris into the plastic asbestos waste bags, preparing boxes, replacing the filters of micro trap machines. Inspection was done on micro trap tents. I used B stairs to get to the 10'" floor.

10"' Floor - 8 workers were present on the floor. Workers were covering the shaft area with poly. 20 micro trap machines were powered on and 5 on stand by.

After following the decontamination procedure I signed the log book and used north hoist'to go back to the ground floor.

Inspection was done on ATC trailer.
