I arrived at the premise at 10:00 pm. LVI schedule informed that activities include:
clear floor, cut ducts, demo walls, ceilings on l” floor.
I proceeded to the decon, signed the log book, don the appropriate level of personal
protective equipment to enter the regulated area on Yfloor. Three workers were present
for the decon maintenance, cleaning the shower chambers and organizing the clean room.
Negative air pressure manometer reading was -0.042.
1®' Floor - 64 workers were present. At the east side vestibule removal of hard drop ceiling
was completed. Nine workers on three scissor lifts were cutting conduit and metal fixtures,
using power tools attached to HEPA vacuum, bringing down pieces from ceiling to the floor;
two workers were spraying the area with amended water. At the west side vestibule
two workers on scissor lift were cutting and removing metal fixtures. The area has been also sprayed.
Three workers were on mezzanine inside the engine room, cutting and removing conduit.
At the various locations workers were clearing shafts from debris, placing debris into the
boxes and spraying with amended water, cutting metal into small pieces, placing into the
boxes and spraying with amended water.
Other workers were: preparing boxes, wrapping beams with poly, loading out sealed and
sprayed boxes through the north side waste decon. A large amount of debris and poly
wrapped beams still exist on the floor.
50 Gaylord boxes were on the floor ready to be taken out. 35 micro trap machines were
powered on and 5 on stand by. Inspection was done on micro trap tents.
I observed that inside layer of the exhaust duct one of the micro trap machines was cut off,
filters of micro traps were dirty; some electrical wires were hanging down to the floor.
Foreman was advised, and he directed workers to repair the exhaust duct, replace filters and
elevate electrical cables in concern.
After following the decontamination procedure I signed the log book and left the decon.
Inspection was done on ATC trailer.
