I arrived at the premise at 6:40 pm. LVI schedule informed that activities include demo,
box out debris on 3"* and 2'’‘*floors.
I proceeded to the decon, signed the log book, don the appropriate level of personal protective
equipment to enter the regulated area on 4*’’ floor. Negative air pressure on the floor was 0.042.
One worker was present for the decon maintenance.
Using B stairwell I proceeded to the 3'‘'floor
3"* Floor - 18 workers were present on the floor. I observed that all micro trap machines were out of power.
Handlers Supervisor and Foreman informed me that as soon as electrical power went off from electrical
closet, they contacted electricians for emergency repair and waiting for repair. Meanwhile workers were
preparing, wrapping boxes, loading out boxes through the waste decon. A large amount of poly wrapped
beams and sealed 15 Gaylord boxes, which have been sprayed with amended water, were at various
locations on the floor ready for load out.
Using B stainwell I proceeded to the 2"'^ floor.
2"" Floor - 18 workers were present inside the work place taking down sheetrock wall in various areas of
the floor, removing floor tiles, removing metals from the ceiling and the wall, cutting to the pieces,
using power tool with HEPA vacuum attached, placing into the boxes and spraying with amended water,
wrapping beams with poly, loading out boxes. Inspection was done on micro trap machines and tents.
10 Gaylord boxes and a large amount of poly wrapped beams were ready to be taken out.
I used B stairwell to go back to the 3^" floor.
After following the decontamination procedure I signed the log book and used south hoist to return
to the ground floor and immediately contacted LVI Supervisor Fabio and TRC Project Scientist
Victor Quizphi to inform them of the 3'“' floor micro traps damage. At 7:30 pm Victor contacted me to confirm
that electrical power was back to normal.
Inspection was done on ATC trailer.
