The Quality Assurance Project Plan (QAPP) documents the methods and procedures by which the
Environmental Community Air Monitoring Plan (ECAMP) operations during the deconstruction
activities at 133-135 Greenwich Street & 21-23 Thames Street (the Site) are implemented and
assessed with respect to quality.
The ECAMP, which is used in conjunction with this QAPP, contains specifics about the numbers
and types of samples to be collected, decontamination procedures, sampling procedures, and all
field activities to be performed during this program.
The Airtek Industrial Hygiene Staff are assigned the responsibility of taking many environmental
samples in support of our clients.
In order to use data gathered during sampling and analysis information about the samples must be
tracked methodically. Sample tracking systems integrate all data and records that relate to one
sample, from the field logs to the final analytical laboratory reports. Sample tracking provides a
mechanism for Airtek personnel to determine the ongoing status of a given sample.
2.0 SAMPLE TRACKING DATA MANAGEMENT
Information gathering and tracking begins before the sample is taken and is completed when the
final laboratory analysis results are accepted by Airtek. When information concerning a sample is
obtained, it is entered into an electronic database. This information is available for review,
generation of reports, or trend analysis. This procedure addresses the following activities and
applies to all environmental samples collected as part of Airtek sampling programs:
• Sample initiation
• Chain of custody
• Sample tracking
• Collection input
• Lab analysis data input
• Data review
• Record keeping
2.1. Definitions
Chain-of- Custody Form (COO: A document that follows the sample from the point of
assembly through sample shipment to final disposition. Custody of the sample is indicated
on the document by signature and dates of possession.
Composite Collection Forum: The document used by the Sample Collection staff to record
field notes regarding composites. This form contains information about the composite
which may not be given to the analytical laboratories such as site location, total sample
volume, and flow through the composite sampler. This form is used only when the
