I arrived at the premise at 10:00 pm. LVI schedule informed that activities include: clear floor, cut ducts, demo walls, ceilings on 1 floor.

I proceeded to the decon, signed the log book, don the appropriate level of personal protective equipment to enter the regulated area on 1 “'floor. Three workers were present for the decon maintenance, cleaning the shower chamber and organizing the clean room. Negative air pressure manometer reading was -0.050.

1®' Floor - 50 workers were observed at the work area. At the east side vestibule I observed three workers harnessed in scissor lift. Hard drop ceiling demo was completed, workers were removing the metal fixtures, and bringing down pieces from ceiling to the floor. One worker was spraying work area with amended water. On the opposite - west side - three workers on scissor lift were removing also metal from the ceiling and spraying the area with amended water.

Two shaft areas were still full of debris and workers were clearing the openings, placing debris into the boxes and spraying with amended water.

Workers at the various locations were cutting metal into small pieces, placing into the boxes and spraying with amended water.

One worker was on ladder re-sealing the hole of the east side critical barrier, other workers were preparing boxes, cleaning floor, picking up debris, loading out sealed and sprayed boxes through the north side waste decon. A large amount of debris and poly wrapped beams still exists on the floor.

55 Gaylord boxes were on the floor ready to be taken out. 35 micro trap machines were powered on and 5 on stand by. Inspection was done on micro trap tents.

I observed that filters of micro trap machines were dirty. Foreman was advised, and he directed worker to replace filters immediately.

After following the decontamination procedure I signed the log book and left the decon.

Inspection was done on ATC trailer.
