I entered the premise at 2:20 pm. LVI schedule informed that activities include: Clear floor, cut ducts, demo walls, ceilings on V floor.

I proceeded to the decon, signed the log book, don the appropriate level of personal protective equipment to enter the regulated area on 1^‘floor. Three workers were present for the decon maintenance, cleaning shower chambers and organizing the clean room. Negative air pressure was -0.023.

1” Floor - 75 workers were present at the work area. Workers on mezzanine and two scissor lifts at the north/west side area were demolishing drop ceilings, clearing the floor from debris, placing into the boxes, spraying with amended water.

Workers on the south/east side of the work area, harnessed in scissor lifts, were Cutting conduits, using power tools equipped with HEPA vacuum, picking up metal pieces and placing into the boxes.

Eagle scaffolding workers were erecting scaffolds at the north/east side work area.

Other workers were; cutting metal into small pieces, placing into the boxes and spraying, loading out sealed and sprayed boxes through the north side waste decon. Approximately 40 Gaylord boxes were ready for load out. 35 micro trap machines were powered on and 5 on stand by. Inspection was done on micro trap tents.

I observed that: 1. Part of live electrical cable needed to be covered with poly; 2. light bulb with melted protection cage was hanging on live electrical cable. Foreman John Rodriguez was advised to conduct emergency repair of poly; he directed workers to re-seal the cable, then immediately contacted electricians to place the light bulb properly. After following the decontamination procedure I signed the log book and left the decon. I met Luis LVI Supervisor and Arnel Javal Senior Project Manager of TRC to share the observation. Inspection was done on ATC trailer.
