Arnel Javal, Sr. Proj. Mgr/TRC Environmental 1:35 pm to 2:30 pm
I entered the premise at 1:35 pm. LVI schedule informed that activities include;
clear floor, cut ducts, demo walls, ceilings on 1 floor.
I proceeded to the decon, signed the log book, don the appropriate level of personal protective
equipment to enter the regulated area on V'floor. Three workers were present for the decon
maintenance, cleaning shower chambers and organizing the clean room.
Negative air pressure was -0.023.
1^' Floor - 75 workers were present inside the work area. Workers on mezzanines of the west
side area were cutting air ducts, metal fixtures using equipment equipped with HEPA
filters, picking up debris, placing into the boxes, spraying the area with
amended water.
Workers on ladder and scaffolds at the north/west side extended area were cutting
vertical beams using tools with HEPA vacuum attached.
At the North side vestibule workers on scaffolds were removing insulation and cutting
air duct, bringing down pieces of metal, spraying with amended water.
Eagle scaffolding workers were at the north side shaft area erecting the scaffolds.
Other workers were: replacing filters, cutting metal into the pieces and spraying the
area; wrapping beams with poly, loading out sealed and sprayed boxes through
the north side waste decon. Approximately 40 Gaylord boxes were ready for load out.
35 micro trap machines were powered on and 5 on stand by.
Inspection was done on micro trap tents.
I observed that exhaust duct, connected to one of the micro traps at the south side
work area was damaged; first main layer was broken and second layer had two holes
on it. I took the photos, then I met John Rodriges LVI Supervisor inside the work area
to advise him to conduct emergency repair. He directed workers, and exhaust
duct was repaired immediately.
After following the decontamination procedure I signed the log book and left the decon.
Inspection was done on ATC trailer.
