1.5.5 Other Required Training and Qualifications Other training and qualification may be required depending on the task work scope and assigned duties of an individual employee. This may include training for waste handling, hazardous materials transportation, respiratory protection, hazardous communication, noise exposure or hearing conservation, and various qualified person requirements (e.g., operation of heavy equipment, forklifts, confined space entry, excavation). Specific additional training and/or qualified person requirements will be identified through the Job Hazard Analysis (JHA) process. The employer is responsible for ensuring that identified training and/or qualified person requirements are met for site workers.

1.6 INCIDENT REPORTING AND INVESTIGATION

1.6.1 Owner Controlled Insurance Program

The WTC Emergency Project will be covered by an Owner Controlled Insurance Program (OCIP). Provisions of the plan will be provided as an appendix at a later date.

1.6.2 Incident Definition

For all Contractor activities, an incident is defined as follows: • A work-related injury or illness • An exposure to a hazardous substance above the allowable exposure limit • Property/vehicle/equipment damage • An uncontrolled fire or explosion • An unplanned spill or release (including air releases) to the environment • A permit or permit equivalent exceedance • Any unexpected contact or damage to aboveground or below ground utilities • Discovery of potential ordnance or projectiles • A “near miss” or an unplanned event that has a reasonable probability of resulting in one of the outcomes described above had the circumstances been different, and for which modifications to management programs will reduce the probability of occurrence or the severity of the outcome.

1.6.3 Employee Responsibility

All contractor personnel have the responsibility to immediately report any incident to their supervisor. Supervisors shall take appropriate corrective action and immediately report the incident to the contractor ES&H manager and the DDC/Site ES&H officer.
