World Trade Center Emergency Project Environmental, Safety, and Health Plan Draft - Revision A 30 September 2001

In addition to immediate verbal (oral) reporting of all incidents to the DDC/Site ES&H officer, for serious incidents, the Contractor shall immediately notify the WTC Emergency Project ES&H Director. A “serious” incident includes the following: • Imminent danger safety violations • Any incident involving the general public or visitors • Discovery of potential ordnance or projectiles • Exposure to a hazardous substance above the allowable exposure limit • Work-related injury requiring more than First Aid • Work-related illness • Spills of hazardous material in excess of 1 gallon or the Reportable Quantity, including unknown or potentially hazardous soils, debris, and containers discovered on roadways. The reportable quantity for petroleum products is 25 gallons. • Potential environmental violation • Any cylinder discovered with unknown contents must be reported to the EPA (646-756- 3136) • Any unexpected contact or damage to aboveground or below ground utilities • Other unusual incidents of a serious nature

1.6.5 Incident Reports

Contractors will prepare a written incident report for all incidents as described in Section 1.6.1 during the work shift when the incident occurred. The report is to be prepared by the supervisor responsible for the Contractor activity/area where the incident occurred.

1.6.6 Incident Investigations

An incident investigation shall be performed for all incidents for which a report is required. The contractor supervisor and the ES&H manager shall perform the investigation and shall include participation by others as necessary. The investigation is to be initiated as soon as possible after the incident.

1.6.7 Investigation Follow-Up

Contractors shall establish a follow-up system for all investigation findings under their control. The system shall identify the corrective action(s) to be taken, the individual responsible for each corrective action, and the date the corrective action was implemented or verified.

1.6.8 Lessons Learned

The WTC Emergency Project will develop lessons learned reports for incidents for site wide distribution as appropriate.
