Inspection Details;

As part of the survey, the Mechanical and the Environmental Professionals inspected the following HVAC system types. A. A total of seven (07) mechanical exhaust fans serving the kitchens and bathrooms and laundry rooms located on the main roof and one (1) passive shaft exhaust located on the main roof. Two (02) mechanical exhaust fans serving the kitchen and the bathrooms located on the 7th' floor roof. Eight (08) mechanical exhaust systems serving the kitchen and the bathrooms located on the 6th floor roof B. One (1) HV unit located on the 16' floor roof and served the elevator room. The system is out of order. C. One (1) AC unit is located on the 16th floor roof and serves thepenthouse # 01. D. One (1) AC unit is located on the 16'H floor serves the penthouse # 02. E. One (1) HV -1 unit is located in the machine room on the 1' floor, serves the hallways l' floor to 16th floor. F. One (1) HV-2 unit is located in the machine room on the 1 floor, serves the rooms corridor and gym in the basement. G. One (1) HVAC unit is located above the ceiling in the coatroom next to the front desk serves the lobby.

All identification data and visual inspections are documented in the attached checklists.

Summary of Findings:

All the mechanical exhaust fans (except the fan # 07 which is serving the laundry rooms) and their associated components and ductworks are covered with WTC related dust. AC unit wtfiich is serving the penthouse # 01. The components of unit and its associated duct need to be cleaned for WTC related dust. All the other systems inspected appear to be free of WTC dust. All the findings are documented in the attached evaluation forms.

Scope of Work:

Clean all the mechanical exhaust fans (except the fan #07 which is serving the laundry rooms) and their associated components and ductworks. The fans are located on the 16th floor, on the 7`s and 6a' floor roofs. A total of approximately 2600.00 linear feet of duct needs to be cleaned. One (1) AC unit and its associated ducts. A total of 30.00 linear feet of duct needs to be cleaned.
