Inspection Details:

As part of the survey, the Mechanical and the Environmental Professionals inspected the following HVAC system types. A. A total of eighteen (18) mechanical exhaust fans serving the kitchens and bathrooms located on the main roof and one (1) passive shaft exhaust located on the main roof. B. One (1) AC window unit located in the elevator room and serves the elevator room. C. One (1) McQuay HV unit is located in the machine roof of the 1' fl behind the mailroom, serves the southeast side of the hallways on the 2d floor through the 9th floor. This unit has not been functioning for the last 15 years according to Mr. Ray Perez (superintendent of the bldg). D. One (1) McQuay HV unit is located on the main floor in the pump room serves the northwest side of the hallways from 2nd floor to 9th floor. E. One (1) Skill Aire AC split unit is located in the pump room, serves the lobby. F. One (1) AC unit is located in the room behind the mailroom and hanging from the ceiling, serves the vestibule by the mailroom. G. One (1) HV self-contained unit is located at the north entrance of the lobby serves the lobby. H. One (1) HV self-contained unit is located at the south entrance of the lobby serves the lobby. All identification data and visual inspections are documented in the attached checklists. Summary of Findines: The mechanical exhaust fans # 2,3,4,5,6,7,8,9,10,11,12,13,14,15,16,17,18 (as they are numbered in the evaluation forms). and their associated components are covered with WTC related dust.

One (1) McQuay HV unit located on the main floor in the pump room. Heavy accumulation of WTC related dusi was observed in the system. A total of 100 linear feet of duct needs to be cleaned.

One (1) Skill Aire AC unit located in the pump room. WTC related dust found in the system. A total of 70 linear feet of duct needs to be cleaned.

All the other systems inspected appear to be free of WTC dust. All the findings are documented in the attached evaluation forms.

Scope of Work:

Clean the mechanical exhaust fans #2,3,4,5,6, 7, 8, 9,10,11,12,13,14,15,16,17,18 (as they are numbered in the evaluation forms), and their associated ducts. The fans are located on the main roof. A total of approximately 2,000.00 linear feet of duct needs to be cleaned. One (1) McQuay HV unit and its associated ducts. A total of 100 linear feet of duct needs to be cleaned. One (1) Skill Aire ACunit and its associated ducts. A total of 70 linear feet of duct needs to be cleaned.
