ACSI (Asbestos Containment Services, Inc.) is writing to provide the following cost
proposal for work at the referenced location. Our proposal is based upon site visitation.

• Remove and dispose as asbestos contaminated waste (ACW) all carpeting and
padding from noe office location of approximately 315 sf.
• Thoroughly HEPA vacuum and wet wipe all surfaces in the affected office including
furniture, books, window silis, personal Items, etc.
• All labor, equipment and materials necessary to provide cleanup as indicated above.
• All work to be performed in accordance with dust control procedures by NYCDEP and
NYSDOi. oertlfied asbestos handlers.
• Disposal of all waste generated due to our work as asbestos containing. Proper
waste manifest disposal documentation will be provided upon completion.
• Comprehensive general liability Insurance and w orlc®rs compensation insurance with
statutory limits.
• Work win be completed in two shifts during day, evening or weekend periods. Shift
one for mobilization, remolvai and cleaning. Shift two for teardawn, waste removal
and demobilization after setisfaotory air ciearance results.

Our total lump sum price for the project is $5 (Five thousand fçijzr hundred
ACSI will require payment at the completion of the work, prior to demobilization of the project.
