On arrival at the site, I met with LVI and TRC personnel who told me about visual inspection of T“ floor. I along with Rammohan of DEP, Mr. Paul Billera of DOL and Mr. Bob Fitzpatrick of EPA entered to the containment area of the L* floor with PPE. There were 20 handlers and supervisors from LVI, TRC and ATC were on the floor ready to help in visual inspection. I found suspect ACM on the joints of the beams and columns in the East side. I called Mr. Fitzpatrick of EPA to check what I found. He agreed and found some more ACM on the same area. Mr. Billera of DOL found some Caulking'on the steps in the west side of the floor. Mr. Rammohan and Mr. Fitzpatrick found foams on top of the brick wall. We all agreed that the 1“’ floor area was not properly cleaned. So it was fail in the final inspection. Negative Air pressure was -0.031. We all exited the work area after decontaminating and signing the log book.

I took the long distance photos of the strips where Rammohan & I found suspect ACM and also workers removing Foams from wall. I used steps to go to basement.

In the basement, I met Mr. Jose Jimenez, supervisor. There were 35 workers working on the floor. They were scrapping fireproofing the ceilings, spraying amended water on the debris and boxing it, loading out boxes, cleaning water fiom the floor, demolishing the wall with the Jack hammer without HEPA. I took photo and told Bovis personnel. The handler attached the HEPA with the Jack hammer. Three bulb cages were opened and without light bulbs. I took photo and told Mr. Luis Castella, supervisor to fix it. Fifty two (52) Gaylord boxes were on the floor ready to be load out. He called the electricians to fix that. Negative pressure was -0.038.1 exited out after decontaminating and signing the log book.
