I arrived at the premise at 5:10 pm. LVI informed that scheduled activities include fine cleaning on the 15“ and 14“ floors, and demolition, boxing debris, cut raceway on 13“ floor.

I entered the decon area, suit up, don respirator and entered containment on 15"’ floor. Two workers were present cleaning in decontamination enclosure system.

15“ Floor - 25 workers were observed on site. Three electricians were found working in electrical room, wrapping electrical wires with poly. Eight workers were on platform cleaning beams and columns in the shaft area, 4 workers were in the process of putting rails to construct the platform between the electrical room and shaft area. Three workers on the west side of the area were cleaning the floor. Five Gaylord boxes were ready for load out. Inspection was done on micro trap machines and micro trap tents - air monitoring pumps were on. 20 Micro trap machines were powered on and 5 were on stand by. I used B stairwell to proceed to the M*" Floor.

14“ Floor - Twenty five workers were present on the floor, cleaning beams and ceiling on the west side of working area. Eight workers on 4 scaffolds were observed cleaning and scraping beams around the shaft area. Additional workers were cleaning the floors around the work areas and vacuuming up the water from the ceiling cleaning. One worker was observed on ladder working with a hammer. Worker was fine spraying amended water in the work area. 2 other workers were found in the process of preparing the boxes. 7 Gaylord boxes were ready for load out. 20 micro trap machines were powered on and 5 were on stand by. Inspection was done on micro trap tents. I used B stairwell to proceed to the 15“ Floor.

13“ Floor-Was no activity or personnel observed on the floor. I used B stairwell to go back to the 14“ Floor, proceeded to the decon, Showered out and signed the log book. I returned to the ground floor using the north hoist. Inspection was done on ATC trailer.
