We respectfully submit our proposal to furnish, erect, dismantle and remove the following inside the first floor lobby:
I. Heavy Duty Sidewalk Shed: 24 linearfeet X 8' wide X14' high legs with wainscot from the deck to the ceiling.
2. Enclosure Around the Stair Tower with Plywood Construction Door and Hasp: 8 ft. X 10 ft. to the height of the shed deck and on two (2) sides to the height of the ceiling.
3. Pipe and Clamp Scaffold: (In the Building Maintenance Service Room) 8'6"X 6’9"X 15'6" deck height with existing ceiling to be removed before installation.
This quotation includes three (28 day) months or less usage.
Cost of the Above 818,900.00 plus 1,013.5 Itax Additional rental (every 28 days) 945.00plus 77.96 tax

This quotation includes standard General Liability andWorker's Compensation tnsurance. It does not include thefollowing: Engineering or drawings not specified above; Shoring if required; Electrical maintenance; Permit renewal; or any items not specifically mentioned above. Relieffrom applicable State and/or Local Sales taxes will be issued upon furnishing ofauthorised evidence oftax exemption only. A 2% service charge per month will be applicable on all past due invoices. Customer is responsible for all collection and legalfees.
