Dear Mr. Weems:

We are pleased to submit herewith our change order #040 to perform the following work at the above referenced premises for the sum of TWENTY SIX THOUSAND FOUR HUNDRED TWENTY SEVEN DOLLARS ($26,427.00).

SCOPE OF WORK As per the engineer’s verbal instructions to weld new rebar to existing rebar intrench with 3" over lap at each end. In order to expose the end if the existing rebar, the concrete has to be chopped away. New rebar and welding $7,800.00 Chop existing concrete $15,000.00 Subtotal $22,800.00 General Conditions 1,824.00 Overhead and Fee 1,477.00 Insurance ..... 325.0.0 Total $26,427.00

Aitacnate If welding of rebar has to be performed on Overtime Add $4,080.00
