We are pleased to submit herewith our change order #112 to perform the following work at the above referenced premises for the sum of TWO THOUSAND TWO HUNDRED SIXTY SIX DOLLARS ($2,266.00):

As per our meeting with Kaback, Preferred Utilities, Silverstein, and Ambassador; regarding the operation of the fuel oil pumps, attached is a letter with recommendations so the oil will flow without any restrictions.

SCOPEOF_WQRK To change the existing anti-syhon valve from the pump suction pipe and install a check valve. Cost $1,950.00 General Conditions $156-00 Overhead & Fee $126.00 Insurance $34.00 Total $2,266.00
