Incident reports and investigation reports will be maintained at the site in accordance with Section 1.6.

1.8.5 First Aid Log

The contractor shall maintain a First Aid Log for any work-related First Aid provided at the Project. The log shall identify the company, project/task, date, employee and short description of the First Aid provided. New entries on the First Aid Log will be reported to the DDC/Site ES&H director in the Weekly Safety Report.

1.8.6 OSHA Form 200/300

The contractor shall maintain a current OSHA Form 200/300 (Log of Occupational Injuries and Illnesses) at the site covering project work. Each recordable injury or illness shall be recorded on this form within 5 workdays. A supplemental record (Incident Report and Investigation Report) is required for each entry and shall be kept with the OSHA Form 200/300.

1.8.7 Environmental Reports

The contractor shall prepare a Weekly Environmental Monitoring Report (WEMR) to summarize the environmental issues and/or incidents that have occurred during the week.

The contractor is also responsible for maintaining adequate records and documentation of compliance with environmental requirements as listed in Section 4 and appendices

1.8.8 Material Safety Data Sheets

MSDS shall be available on-site in the work zones prior to and during work activities for ready access and review by site workers.
