This is in reference to your recent conversation with Dennis White,
V Joel A Miele Sn, RE. i = Supervisor Training and Certification Unit, regarding asbestos investigator Keimy
Lee. As per your request, the results of the audit of Mr. Lee’s asbestos survey and

Bureau of Aln Nolso^ t^; On March 12, 1999, the Asbestos and Lead Control Program (ALCP)
Hazardous Materials ^ / received a letter from your office requesting information from Mr. Lee’s training
(718) 59&44iir^^^^ and certification file. As you may be aware, after clearance from your office, the
<z ALCP scheduled and performed the audit on January 27,2000.
The audit of Mr. Kenny Lee’s records disclosed that he had performed
investigations and completed “Not an Asbestos Project”, ACP 5 forms, during a
lapse in his certification status. In accordance with Title 15, Chapter 1 of the Rules
of the City of New York (RCNY), an individual must be a certified investigator on
the date of investigation (ACP5 Form, line 11) and on the date the form is signed
(ACP5 Form, line 16). Mr. Lee was issued seven (7) notices of violation citing
