Attachment - A 8 Inch Water Main Break - 97th Street Between 157th and 1581h Avenues; Queens _ At 2:00 p.m., Monday, November 27, 2000, we received a report of a water main break in 97th Street between 157th and 158th Avenues. DEP personnel responded, were on location at 2:30 p.m. and found street flooding with a heavy flow of water emanating from a broken 8 inch water main in front of 157-43 97th Street. The Police Department and the Mayor's Office of Emergency Management were on location; 97th Street was closed to traffic. DEP staff initiated a shutdown on the eight inch water main. They experienced some difficulty closing the required water valves and did not get a complete shutdown until 7:00 p.m. Approximately one hundred and fifty residential consumers were affected. Department personnel repaired the water main and water service was restored at 4:00 a.m. The damaged roadway was repaired and 97th Street was re-opened to traffic.

12 Inch Water Main Break - East 175th St. Between Mohegan Avenue & Vineyard Place; Bronx On Monday, November 27, 2000, at 2:10 p.m., the NYC Fire Department reported a possible water main break in East 175th Street. DEP personnel responded, were on location at 3:00 p.m. and found a heavy flow of water emanating from a broken 12 inch water main in front of 855 East 175th Street. The Mayor's Office of Emergency Management and the Fire Department were on location. The crew called for assistance and initiated a shutdown on the 12 inch main in East 175th Street that affected consumers from Crotona Park North to Marmion Avenue (a four block area). The water was shut off at 7:30 p.m. Due to street flooding and roadway damage, traffic was diverted off East 175th Street. DEP personnel repaired the water main and water service was restored at 1:00 a.m. on Tuesday. The site was turned over to the emergency repair contractor and reconstruction of the damaged roadway was completed on November 30.

Water Service Break - East 133rd Street Between Bruckner Boulevard and Cypress Place; Bronx At 1:30 p.m., Monday, November 27, 2000, DEP Central Communications received a report of a water main break in front of 158 Bruckner Boulevard. DEP personnel responded, were on location at 2:30 p.m. and found considerable street flooding. The crew determined that the break was in East 133rd Street and shutdown the 12 inch water main in East 133rd Street between Bruckner Boulevard and Cypress Place; the water was shut off at 5:25 p.m. Con Edison was notified and responded to ensure that the integrity of their 30 inch high pressure gas main had not been compromised. Department personnel excavated over the defective water main and determined that the break was on the service line to 608 East 133rd Street. The 4 inch valve on that line was closed, stopping the leak, and the building owner was notified to make repairs. The water service to other consumers on East 133rd was restored at 7:00 a.m., Tuesday, November 28.

12 Inch Water Main Break - West 21st Street Between 9th and 10th Avenues; Manhattan At 2:11 a.m., Tuesday, November 28, 2000, the NYC Fire Department reported a water main break in West 21st Street between 9th and 10th Avenues in Manhattan. DEP personnel responded, were on location at 2:30 a.m. and found considerable street flooding and roadway damage.
