Dear Mr. Weems'

We are pleased to submit herewith our change order #055 for the following work at the above referenced premises for the sum of THIRTY FOUR THOUSAND NINE HUNDRED FIFTY FOUR DOLLARS ($34,954.00):

SCOPE OF WORK In the loading dock and lobby areas where the fuel oil lines were to be installed below the concrete slab, the electncian had to fix alt the conduits damaged by the chopt^ng of the concrete. These conduits were never indicated on the drawings to be in that location.

In order to fix these conduits, they had to be traced out for all the circuiting. New conduits and wire had to be installed.

Additional chopping was performed to expose the existing steel beams and plates that were interpreting with the new fuel oil lines so the engineer could examine them.

The following are the costs for the above work.
