* Mr. Kupferman _ approached a female with a child on a stroller and gave her the TEM sample analysis result report and PI1 Laid to the individual " There is asbestos in the elevator shaft and on the windows". The female individual then approached me and the EPA Officer and identified herself as! PII and asked that if we could inspect her. partment at 132 Duane Street apartmer _Pii_nd want to discuss the issue at 150 Franklin Street. I inspected'._._._.__P .i._._. s apartment and there was no visible dust observed in the apartment. She was concerned about the construction at the main labby. The lobby had a new ceiling and entrance door and frame installed. No suspect ACM debris observed. Fiberglass was installed.on the ceiling prior to sheetrock installation.; PI I -claimed that she sends her 2 year old child to the apartment PII at 150 Franklin Street and it was not a licensed day care center. The samples were analyzed by PLM method and there was no ACM detected.

* The EPA Officer contacted me the next day at 9:14am (4/18/02) and claimed that his samples from the roof and on the window ledges contains ACM based on TEM analysis. I then immdediately contacted and left a message for the Co-op Board President for 150 Franklin Street, Ms. Emily Sarkin and informed of the inspection conducted on 4/17/02 and the findings. At 10;34am Ms. Serkin returned my call. I explained her of the details of the inspection on 4/17/02 and advised her to contract a DOL licensed Abatement Contractor and an Air Monitor to perform the clean-up of the roof and the elevator shaft expeditiously. She stated that she resides at Westchester and will do everything to hire an abatement contracter. Ms. Serkin also stated that the occupant of apartmen(Pii i.had history of complaining for everything and always disagreed with the Board and tried to stop any work in the building. At mid day I was contacted by Tim Donohoe of Donohoe Environmental and explained him of the clean-up procedure and areas. (The entire roof and elevator shaft including window ledges with TEM air samples for clearance in every apartment). A scope of work was received and the clean-up work was scheduled to start at 4pm on 4/18/02.

* The same afternoon an individual claimed to be the assistant to Joel Kupferman called me and said that the clean-up work is scheduled to start that evening and they intend to stop the job. I advised him that no one will be allowed to stop the clean-up job and was approved by DEP and US/EPA and will be monitored by the agencies.

* I came to the building at 8:30pm to inspect the clean-up activities. At this time the roof was completely cleaned and the results were not known. The workers were cleaning the elevator shaft. Handler Supervisor Fidel Castro, Cert#83128 and three handlers fo DSA Services the air sampling technician Latisha Matos, All 01-18793 were present at site. Tim Donohoe, The principal of Donohoe Environmental was also present on site overseeing the clean- up actvities. I then departed from the site after my inspection.

* On 4/19/02 morning I had contacted Tim Donohoe to get an update of the clean-up and he stated that the elevator had brokedown at midnight and was fixed only in the morning and the crew will go back to perform the clean-up and the elevator was blocked from usage by tenants. TEM results of apartments and inside the elevator shaft (PCM) air sample results were below clearance level. A visual inspection was conducted by me and The EPA Officer at 3:10pm and still observed minor dust in the cracks of the window ledges in the elevator shaft. I contacted the contractor and the consultant advised further recleaning of the window ledges. The EPA Officer departed from the site at 4pm 4/19/02, Friday. While I was waiting for the crew several of the occupants_of 150 Franklin Street offered if anything needed(like power). I spoke to the female individual who babysits in aj.P!!_, who claimed that she just babysits some of her friends children and do not have a license for a daycare center. The abatement crew arrived on site at 4:30pm and re-cleaning of window ledges in the levator shaft resumed. Re-cleaning completed and aggressive TEM clearance air-sampling started at 7:30pm and I departed from the site.

* 4/20/02 TEM aggressive clearance air sampling results were below clearance criteria in the elevator shaft. The entire clean-up was completed.
