Request for a Change Order, Additional Work Schedule

Dear Ms. Theoderellys:

JLC Environmental Consultants, Inc. (JLC) appreciates your continued consideration for the provision of specialized Asbestos and HAZMAT Services. We would like you to process a request for payment for additional work. Our original bid covered the work for a period not to exceed three weeks (see attached addendum #2). This consideration entitled the monitoring of the removal in that time period. However, the removal has taken longer.

Additional hours and resources were spent; as a result there was a duplication of work. If the work had taken place in the allotted time period no additional man hours and air monitoring tests would have been required, since the work would have been part of the over-all project. However, the second mobilization required the duplication of work in terms or man-hours and air monitoring tests. These additional resources were not part of our original Lump Sum quotation.

I would like to present for your examination the following break down for the additional mobilization:
