INSPECTION DISCLOSED: 9/25/02: Jan Wolkovicz and five handlers and Jesron Garcia of Warren&Panzer came on site. At 11:30am I rode on the North side scaffold to do a visual estimation and inspection on Liberty Place(the Alleyway). Inspection revealed that cleaning requires from 281h floor(the maximum the scaffold can go up) and down on window ledges and facade details. Some windows were new and clean and some were dirty. The north scaffold is up and began cleaning. The second scaffold was not up until 3:45pm due to power problem. With the power only one scaffold can be operated at a time. Power cables were switched at times. It was raining and both the scaffold were down up to 9th floor. 9/28/02: The same crew continued facade cleaning. Work was delayed due to safety lines struck and jammed between facade and metal cable. Work was stopped at 7pm. 9/29/02: The same crew continued on the scaffold. At 8 pm North scaffold was at 2' floor•and the south scaffold was at 61h floor. 10/04/02: The same crew was on site. I rode on the north scaffold at 9:30am up to the 28`h floor and some areas required recleaning. Then I inspected the south scaffold and also required re cleaning and was completed.at 12:30pm. 10/05/02: On Nassau Street street side facade cleaning began on two scaffolds by the same crew. 10/06/02: Cleaning continued until 8pm and visual inspection was not possible due to poor visibility. 10/13/02: Jan Wolkovicz and 5 handlers on site. Ramraj Sethram of W&P performed air sampling. Three handlers were began cleaning the facade on Liberty Street. I conducted visual inspections on the Nassau side facade and recleaning was done by two handlers and was cleared at 4:30_ pm. I also inspected the facade on Liberty Street side and cleaning was compled up to 11th floor. 10/14/02: Today was a holiday and the crew continued cleaning on Liberty Street facade and inspection was performed and cleared by Inspector Dennis Antzalatos in my absence.

The north side facade required acces to the roof of the Pizza Parlor and the owner refused acces due to previous dispute with 55 Liberty management. Cralstein Lutchmedial pursue for a commissioner's order. In the meantime, the building management dealt with Trio to use the scaffolds to perform their own structural inspection for another two weeks. Each time after DEP clearance the plotforms were properly secured to a structure of the building. During the removal of cables on the last scaffolding the building management claimed that they cannot let Trio or tlieir' sub contractors in to the building. Management claimed that they suspect the rigger of jammimg the floats of the water feeders. Thre was no witnesses to prove from the management's end. A Commissioners Order was issued to the Pizza Parlor owner and he complies. The North facade clean-up project was awarded to Fiber Control, Inc. from zone/.
