Dear Mr. Weems;

We are pleased to submit herewith our change order #085 to perform the following work at the above referenced premises for the sum of THIRTY FOUR THOUSAND FOUR HUNDRED ONE DOLLARS ($34,401.00);

SCOPE OF WORK

While laying out the route of the fuel-oil lines, the building’s elevator inspector from Otis was in the elevator pits. We showed him the route the fuel-oil lines were taking, and he informed us the route would not work. When they test the elevators, the counter weights would interfere with the piping. Therefore a new route has to be taken and the following additional work has to be performed in order to install the piping;

Masonry - Remove and reinstall block work around two (2) doors. Open two (2) holes in masonry between elevator cars 23, 22 and 1. Replace block upon completion. $5,200.00

Carpentry - Reset two (2) existing doors and frames $900.00
