Aliotta 5576CL The Owner agrees and hereby authorizes Swanke Hayden Connell & Partners to X Proceed with Additional Services X Proceed with revised Scope of Services X Incur Out-of-Pocket Expenses As follows: DCAS requested the revision of this additional service authorization on 06.18.99. Per the OEM/DCAS meeting on 03.15.99 with Shen Milsom & Wilkie, Inc. & SHCA, OEM verbally requested a fee proposal for additional Audio- Visual scope of work to expand the RF Distribution (Broadband) system to include approximately 200 more feeds as well as otlier miscellaneous items. This proposal is specific to tlie services listed below for the expansion of the RF Distribution System only. The scope of design services includes architectural coordination with the Audio-Visual Consultant to implement the requested scope of services. The scope of services is as follows; 1. Conduct user interview(s) to determine exactly what feeds are required, where they originate from and in what format they are available. 2. Prepare a report and budget estimates tirat describes the systems functionality. 3. Meet with users to secure agreement and written approval as to functional requirements prior to proceeding with the design. 4. Review the existing design per the installed equipment to determine how tire new scope of work will interface with the existing system conditions and determine modifications required to accommodate new scope of work. 5. Prepare design documents (drawings and specifications) as required. 6. Review A-V contractor’s bids. 7. Review successful A-V contractor’s submissions, shop drawings, etc. 8. Provide periodic site visits to monitor the progress of work and attend site meetings as required.______________ Compensation to the Architect shall be adjusted as follows: Fee; $ 10,500 Out-of Pocket Expenses: $ 1,000 (Not-to-Exceed) Time for performance shall be adjusted as follows; N/A Upon return of a fully executed auihorization, this service shall become part of the Agreement identified above. In the event tliat, after transmittal of this Agreement to the Owner, payments to the Architect are made by the Owner after performance of any of tite services called for herein, the Owner will be deemed to have accepted all tenns of the Agreement not otherwise objected to.
