The following DEP staff worked at 110 Maiden Lane, NYC, after 9/11 during the dates shown below (per my memo of 10/13/2004 to them and to you). PH i have responded to he Survey as follows (I can give you their responses, if needed): Pll worked near the WTC from 9/18/01 to 11/16/01. I have not seen a doctor .’hoarding my work there. I may call the telephone number given for tomorrow's Seminar, 1 )/14/06, at Barclay Street for information. !PH i - worked from 9/18/01 until 11/16/01. He has not seen a doctor related to work near WTC. He has returned the Survey and said he may cal.I the information number given for the Semi'nar on Saturday, 10/14/06. Pll worked near WTC from 9/18/01 to 11/16/01. She ^ha.s...visited the World Trade istry at Mt. Sinai Hospital re her working near the WTC. ..PH__ihas returned the Survey and has called the number for tomorrow's Seminar for information.

PH A- worked near the WTC from 9/15/01 to 11/03/01. I do not know whether vern ha.s visited the World Trade Registry at Mt. Sinai. I have not heard from her garbing thi.s Survey.

PH H- worked near the WTC from 9/15/01 to 11/16/01. I believe he has visited Mt. cLT'Tegard1ng his work near the WTC. I don't know whether he is attending the Seminar morrow; I have not received a response to the Survey I sent him; he probably gave it to h.n Leonforte. PH (She now works for HRA.) PH I worked near the WTC one-two days a week, from 9/18/01 to 11/16/01. I left information for her at her home telephone (could ■t set through at HRA) about the Survey and the Seminar scheduled for tomorrow, 10/14, -.h the information number, should she possibly be worried about becoming ill in the a are. hope this information will be helpful to you for completing the Survey for the bureau.

appears that most of the questions will have to be filled out by personnel, however, I .Id like to provide an answer to lA, la and b. . is important that staff who were at the site be included in the survey and anything at; might come out of the review panel.
