Topic: Laboratory Reports and Chain of Custody Forms to be corrected ********************************************************************************** The following laboratory reports and chain of custody sheets have been reviewed and found to be incorrect: ATC Group # s:
1) 8043 - The chain of custody has “Building 49-A.” It should read “49 Ann St.” 2) 8050 - The address on the report reads “185 BRDWY” and the address on the chain of custody reads “183 BRDWY.” 3) 8200 - The chain of custody reads“122 Fulton Ave.” It should read “122 Fulton St.” 4) 8271 - The project address on the report and the chain of custody does not match. 5) 8282 - The address on the report should read “114/116 Fulton Street.” 6) 8314 - The chain of custody is missing information in the bottom left box. 7) 8308- The address should read “7 Dutch St.” 8) 8320 - The address should read “ 109 Reade St.” 9) 8326 - The chain of custody is missing information on the sample locations. 10)8338 - The report reads “30 Wall Street, Side Win Dems.” Please check with Air Tech for verification. 11) 8352 - There are no dates recorded in the bottom left box on the chain of custody. 12) 8353 - No stop flow rates are recorded. 13) 8365 - The address should read “67 Greenwich Street” on both chain of custody and report. 14) 8380 - The report is missing the address. 15) 8382 - The report is missing the last digit in the sample numbers.
Please have these reports and chain of custody sheets corrected and resubmit by July 12,2002 with affidavits explaining the reasons for the revised reports and chain of custody sheets. I still have not received affidavits for the earlier incorrect reports and chain of custody sheets (see memo dated June 25,2002. Please submit these affidavits by July 12,2002.
