1) 8177 - The chain of custody has “Beeckman.” It should read “Beekman.” 2) 8111 - The report has “ 116.” It should read “116 Nassau St.” 3) 8092 - The report and chain of custody has “160 Nassau St.” It should read “116 Nassau St.” 4) 8091 and 8103 - These reports are missing the address 23 Trinity/47 Broadway. 5) 8043 - The report and chain of custody has “Building 49-A.” It should read “49 Ann St.” 6) 8144 - The report is missing the address 156 Chambers St. 7) 8105 - The report is missing the address 118-120 Fulton St. and missing results for samples # 04 and 05. 8) 8253 - The chain of custody is missing a large part of information. This seemed to be a problem with the fax machine. Needs to be faxed again. 9) 8217 and 8198 - The chain of custody sheets have “TEM EPA LEVEL II circled. It should be “TEM AHERA.” 8198 - The address on the chain of custody states “5 Beeckman.” It should read “5 Beekman.” The report is missing the address. 10) 8200 - The report and chain of custody states “122 Fulton Ave.” It should read “122 Fulton St.” 11) 8216 - The report is missing the last digit in each of the 5 sample numbers. 12) 8246 - The report is missing the address “114 Liberty St.” 13) 8231 - The report is missing the “0."
Please have these reports and chain of custody sheets corrected and resubmit by June 30,2002 with affidavits explaining the reasons for the revised reports and chain of custody sheets.
