2/21/02 ACP attempted inspection of Apartment . PH. J Visited the Building twice L____ ph.___ .i was not at home even though she had an appointment. The second visit the doorman said she was home but was not answering. In addition, the phone number came on as a fax machine.

2/20/02 Complaint 412235 received

2/19/02 Complaint number 411791 received. Complainant was concerned about dust and fibrous glass. An appointment was scheduled for inspection. Unable to perform the inspection (resident was not available and had left telephone number for doorman rather than residence)

2/15/02 ACP Inspection of Apartment L PH i . Inspection revealed that there was no visible WTC.

2/14/02 Complaint 410716 received from apartment PH Resident List of air monitoring firms faxed, inspection scheduled.

2/12/02 DEP sent letter requesting copies of environmental hazard assessments and a summary of clean up activities.

2/11/02 Complaint 409645 Received Request for air test, I PH 2/8/02 DEP sent a letter to Milford Management indicating that visible accumulations of debris were observed on the roof and setbacks, the need to assess the material, and to perform appropriate clean-up.

1/28/02 DEP performed an exterior survey of the building and visible debris on the roof and 9* floor setback

3/06/2002 .j- Inspection revealed that there was no visible WTC debris in the apartment.! PH wanted to have the air in the apartment tested. She was faxed a copy of various'^ monitoring contractors.
