RE: ASBESTOS AIR MONITORING PERFORMED AT: 115 BROADWAY, MEN'S STORE, NEW YORK, NY
Dear Ms. Kung:
From October 8th through October 11th, 2001, Consulting & Testing Services, Inc. performed asbestos air monitoring at the above referenced location. COMCO, asbestos contractor removed loose debris and the affected area was HEPA vacuumed and wet wiped. Enclosed, find the following documents:
1. CTSI's Daily Log Form 2. EMSL's PCM Certificates of Analysis
• Air monitoring was conducted by CTSI. Air samples were collected during the clean up phase of the project and analyzed using Phase Contrast Microscopy (PCM) method. Air samples taken passed clearance criteria per applicable regulations (<0.01 f/cc).
The project was completed successfully. All applicable Federal, State and Local Rules and Regulations were strictly enforced during removal.
