CONTACT PERSON Ravi Dindayal, Chief Engineer n OF INSPECTION FROM: 8 am _5 pm_ INSPECTION DISCLOSED: 10/26/02 : Jan Wolkovicz and Five handlers began setting up the decon on the roof and began cleaning various spots of the roof. Air monitoring was perfomed by Warren&Panzer. 10/27/02: The crew continued the cleaning in the roof and completed the roof at the end of the day. 11/30/02: Jan wolkovicz and six handlers came on site and setup scaffoldings on the 10th floor setback. Decon was setup at 10th floor for the exterior facade clean up,Two windows and ledges fo two facades facing east and west were completed. Oghogho performed air sampling. 12/01/02: Jan Wolkovicz and four handlers from Trio and Oghogho Erhaber of W&P on site. A large scaffolding was utilized to clean window s and ledges. At the end of the day that facade facing pine street was completed. 12/14/02: Jan Wolkovicz and six handlers and Festy Osemvegie of W&P were on site. Two scaffolds on Nassau Street were up on the facade for windows and ledges cleaning. Cleaning started from the 18th floor since cables were dropped from the vacant 19th floor. Debri observed only from 19th and down on ledges. Cleaning was done up to 8th floor at the end of the day. 12/15/02: The same crew came on site and continued cleaning and the cleaning was completed at the end of the day for that facade. 12/21/02: Jan Wolkovicz and six handlers and Festy Osemvegie were on site. Two scaffolds were ready for the facade of the pyramid building on wall street. Handlers began cleaning window and ledges from 18th floor and down. Cleaning was done up to 7th floor at the end of the day. 12/22/02: The same crew continued the cleaning on facade. At the end of the day facade cleaning was completed and the scaffolds were dismantled. Crew vacuumed the ledges of 19th floor on Wall and Nassau Street sides. 12/30/02: Jan Wolkovicz and four handlers came on site to clean the facade west of main entrance on Wall street from floors five and down where debris observed. A Genie manlift was utilized and the work was started at 6pm and completed at 12:30am. This completes the project in the entire building.
