We are [pleased to submit herewith our change order #035 to perform the following work at the above referenced premises for the sum of FIVE THOUSAND SEVEN HUNDRED NINETY SIX DOLLARS ($5,796.00) for the following work.

As per memo fax from David Litvin dated November 5,1998 requesting the termination of the fiber optic cables, including the furnishing of the patch panels. Cabinet supplied by others. Subtotal $5,000.00 General Conditions 400.00 Overhead and Fee 324.00 Insurance _.__72.00 Total $5,796.00
