6.3 REPORTING PROCEDURES

6.3.1 Investigation and Reporting of Accidents

Each entity will conduct its own accident investigations. Information pertaining to accident reporting and investigations should be made be available to the employees of each entity. Immediate notification will be made to the agency’s safety officer when an injury or illness results in a fatality, permanent total or permanent partial disability, hospitalization of any employees, or an accident in which property is significantly damaged.

6.3.2 Exposure Reporting

P&J will prepare and provide to all entities at the 11:00 am safety meetings periodic reports that summarize the findings of air sample and exposure monitoring data. Other agencies and entities are encouraged to provide any data they are collecting to the others. Results of air sampling data will be available to site personnel, upon request.

6.3.3 Maintenance of Data Logs

Records of exposure data, accident reports and field logs will be maintained for the duration of the project by each entity.

6.4 EMERGENCY RESPONSE

The FBI has an onsite medical staff located near the mess tents that can provide immediate response to medical related emergencies. A heli-pad is constructed on site for easy access to Medivac if necessary. An EE&G M.D. and medic will be present in the PPE Sprung Structure during shift changes.
