This information shall inc^^^but is not limited to, actual sampling date and time, exterior evaluation (pass/fail), type of HVAC s> stem (central, individual or none), HVAC impacted (Y/N), occupant present (Y/N), written access authorizations (Y/N), EPA approval (Y space occupied (Y/N), mold present (Y/N). Dos and times shall be associated with all activities including start and finish date.
