This letter is in ref1 rence to the Department of Health's proposed contract with East Harlem Council for Human Services. As we have discussed, we are eager to resolve any outstanding issues that may affect the start up of this veryimportant asthma, prograttt.
As you may already know, Ms. Elizabeth Sanchez, Chief Executive Officer of East Harlem Council for Human Services, Inc., was investigated by the United States Attorney's Office for the Southern District ofNew York. As insult ofthis investigation, Ms. Sancheawas charged in January 1998 with impersonating a federal agent. In October 1998, Ms. S~fttchez pled guilty to theft of government funds. t `
During the time these charges were pending, Ms. Sanchez failed to disclose this information on the City's vendor Information Exchange System ("VENDEX') Questionnaire filed with various City agencies. When Ms. Sanchez appeared before the New York City Depat4nerit of Investigation on March 15, 2000 to discuss the VENDEX omissions, she explained that she signed the VENDEX Ouestionnaires without eitini S the responses.
The Department is now attempting to make a determination as to your organization's responsibility, which includes an assessment of the integrity of the organization and its officers. Although the evaluations that the Department has reviewed for East Harlem Council on other city contracts reveal very good ratings, the Department cannot overlook Ms. Sanchez's actions.
