Cook County Study, Threshold Jail Program, 1999

• Program: Case management, medication monitoring, housing, transportation, money management post-release, and continued contact throughout subsequent arrests and hospitalizations.

• Agencies involved: Illinois Office of Mental Health; Cook County Jail.

• Subjects: Referred from jail. All have "three strikes against them." All with mental health diagnoses, most with substance abuse, histories of non-compliance. 30 clients studied.

• Results: Clients' records were reviewed for one and two years before they entered the Threshold Jail Program and compared to one and two years after being in the program.
