CLASSIFICATION RATE • Skilled Cleaning Laborer (Straight Time) • Restoration Area Supervisor (Straight Time) $30.00 per hour • Project Coordinator (Straight Time) $45.00 per hour • Project Manager (Straight Time) $60.00 per hour $75.00 per hour • Senior Project Manager (Straight Time) $100.00 per hour • Straight Time Hours • Overtime Hours (Time-and-a-Half Rates) Weekdays 8:00am to 4:00pm Weekdays 4:00pm to 12:00am; • Saturdays 8:00am to 4:00pm Premium Time Hours (Double-Time Rates) Weekdays 12:00am to 8:00am; Saturdays 4:00pm to 8:00am; • Sundays & Holidays Travel/Staging Time Charge One (1) hour charged per shift • Materials & Basic Supplies Ten percent (10%) of all Labor Charges • Ozone Generator (High Capacity) • HEPA Vacuums (High Capacity) $275.00/day per unit • $150.00/day per unit Atmospheric Deodorization $0.02/cu. ft. (per application) • Antimicrobial Surface Treatment $0.25/sq. ft. (per application) All materials are guaranteed to be as specified, and the work is to be completed in a workmanlike manner for the total sum of: PRICE NOT TO EXCEED: $I Q? (plus applicable sales tax) PAYMENT TERMS • Buyer agrees to make payment to MAXONS for services rendered upon amounts and on the terms specified above, regardless of whether completion of the work specified hereunder in the Buyer or another person or entity is involved and regardless of whether Buyer is entitled to reimbursement for such costs from his or from some other person's or entity's insurance carrier. If payment is to be made directly by an Insurance company, client agrees to make payment in full within 24 hours of receipt of any insurance check or draft related to this claim.
