Re: Clean up of asbestos containing dust and debris on Roof of 115 West Broadway, Manhattan

Square Footage of Roof Clean Up Area: 3,000 Sq. Ft.

Starting Date/Time: 8:00 am, November 16, 2001

Scope of work:

J. C. Broderick and Associates performed the asbestos survey and analysis of settled dust samples on the roof (enclosed). The roof will be cleaned by Branch Services as an asbestos abatement project in accordance with all the applicable regulations ofN.Y.S. Industrial Code Rule 56 and N.Y.C. Title 15.

A showering unit will be set up on the roof. We will clean the roof using H.E.P.A. vacuuming and wet wiping methodology. All asbestos containing debris will be wet with amended water, picked up and double bagged in 6 mil polyethylene asbestos bags and transported to a D.E.C. approved disposal site. A copy of the landfill waste manifest will be mailed to the oviners upon our receipt. J. C. Broderick and Associates will act as the independent third party consultant laboratory to perform air testing in conformance with New York City and State Law.

All workers will have all necessary and required New York State, New York City and Federal licensing for their respective duties on the asbestos abatement project including but not limited to asbestos supervisor, asbestos handler and asbestos project designer. Please call me with any questions.
