Topic: Laboratory Reports and Chain of Custody Forms to be corrected

The following laboratory reports and chain of custody sheets have been reviewed and found to be
incorrect:
ATC Group # s:
1) 8400 - The chain of custody has “PCM 7400” circled instead of “TEM AHERA.”
2) 8423 - The chain of custody needs to be faxed again due to misfeed.
3) 8426 - The chain of custody seems to reads“10 Cordlandt” and the report reads “ 10-12 Courtlands
Street.”
4) 8411 - The lab report is missing results for samples #11,12 and 13.
5) 8457, 8416, 8445, 8493 - The chain of custody sheets and lab reports reads “156 Williams Street
instead of “ 156 William Street.”
6) 8482 - The address on the lab report reads “60 Finest” It should read “60 Pine Street.”
7) 8483 - The address on the lab report reads “ 60 Pune Street.” It should read “ 60 Pine Street.”
8) 8447,8449 - The address and report list “ Read St” instead of “Reade St”
Please have these reports and chain of custody sheets corrected and resubmit by August 2,2002 with
affidavits explaining the reasons for the revised reports and chain of custody sheets. The deadline for
submitting items requested as per the Memo dated July 5,2002 will be extended to July 29,2002.
Please be advised that failure to submit requested documents and revised chain of custody and lab report
sheets and may delay the processing of invoices.
