Based on the audit by George Anakhu conducted on 04/19/02 there were 2 sites where George did not see the air techs and did not see any air sampling being conducted. This conflicted with the chain of custody forms that were submitted with the lab report for that day. The chain of custody forms showed that air sampling was being done at the time George was at the 2 sites. Arrangements were made to inspect all 20 sites with the Project Manager from Warren and Panzer. Greg Fruchtman, George Anakhu and I went to the site on 04/24/02. Based on the inspection I concluded that the 2 sites in question were in fact sampled. The exact locations were confirmed by the Project Manager. The locations where George had done his audits were in close proximity to the sampling sites but the techs had set up their equipment behind barriers. All sites were found to be acceptable and no further actions will be taken.
