Dear Building Owner/Management:

It is our understanding that your building may be eligible for participation in the World Trade Center Indoor HVAC Dust Cleaning Program administered by the USEPA.

In order to be eligible for the program, the following information must be submitted to the New York City Department of Environmental Protection within seven (7) business days:

• Copies of insurance claim forms, responses from your insurance company, settlement and payment agreements, and payment receipts for cleaning activities. • A description of the impact of September 11, 2001 on your building and building systems. If your building was not impacted, please state so in your submittal. • A description of the clean up activities prior to and during re-occupancy. • A description of the building’s heating, ventilation, and air conditioning (HVAC) system(s) and information regarding HVAC cleaning activities that were performed after September 11,2001. • Documents regarding sampling and cleaning of the HVAC system, including but not limited to the work procedures, the name of the company that performed the cleaning, invoices, plan proposals, etc.

Participation in the program requires an authorized building representative to complete the request form and sign the agreement. In accordance with the agreement, the building representative must agree that insurance payments and/or other compensation which was received, or will be received in the future. for activities covered by the Program, and which has not been spent for such piuposes. will be paid to the NYCDEP.
